Medical reports stating health issues
Marriage, divorce, custody, death, adoption, deed poll, deferment from MOE etc certificates/papers
Kindergarten Report Slip
For application made by guardian, please attach: a) an authorisation letter from the parents; or b) the Marriage Certificate of step parents (where applicable)
